By submitting a photo for “Faces of Food Allergies,” I consent to the unrestricted use, by Kids With Food Allergies Foundation, (and those acting with its permission and authority), of any and all photographs submitted, in whole or in part, unlimited use, for all purposes in any form or medium, including, without limitation, its use through or on any electronic media, including the Internet.

I waive any right to inspect or approve the finished product or products or the advertising copy or printed matter that may be used with the finished photograph(s).

I hereby release Kids With Food Allergies from any and all claims in connection with the photographs(s), including any and all claims of libel.

I understand that this consent is given without expectation of compensation.
I am over age 18. I have read and fully understand its contents
I am the parent or guardian of a minor. I have read the above and fully understand its contents. I hereby grant permission for my child's/ward's photographs to be used in the manner specified above.
